I acknowledge that parents/guardians understand that music, movement, dance, and face painting activities involve light physical activity and accept responsibility for their child’s participation. FLYNOTES, Krysta Ferrara, and affiliated hosts/venues are not liable for injuries, accidents, lost items, or damages that may occur during the event. Parents/guardians are responsible for remaining present and supervising their cduring face paint and musical activites at all times.

I agree to pay non-refundable $150 deposit (off of event price) to secure the date and time of event and understand the booking will not be secured without payment.

Ms. Krysta will review your submitted information within 48 hours and create an invoice that will be sent to the email address provided above. A deposit is required upon receipt of the invoice to reserve and hold your preferred event date and time.
